A weak link anywhere in the chain can undermine confidence in the whole rail.

UPI’s biggest risks come from its combination of instant settlement, broad interoperability, and user-driven authorization. That makes it attractive to fraudsters, because a successful social-engineering event can move value quickly before the victim or institution has time to react.

Failure mechanism: Attackers commonly exploit trust in legitimate apps, spoof payment prompts, SIM-swaps, device compromise, or misleading requests for approval to induce unauthorized transfers. The payment rail itself may be sound, but the surrounding human and device trust layers are easier to attack than the bank ledger.
