What are the signs that an organisation’s authentication controls are not working as intended?

Warning signs include repeated authentication-related breaches, continued use of insecure 2FA methods, and little or no change after an incident. How to spot a control problem, not just a bad login day

Authentication controls are usually failing when the organisation keeps seeing the same access pattern break in the same way.
