Risk Review

← Back to Glossary
By NHI Mgmt Group Updated October 25, 2026 Domain: Governance, Ownership & Risk

Risk Review is a formal check of whether an identity, system, or change introduces unacceptable security, compliance, or operational exposure. In IAM and NHI governance, it evaluates access, secrets, control gaps, and business impact, then records mitigation, approval, or rejection before deployment or continued use.

What a Risk Review Actually Evaluates

A risk review is not just a label for approval. It is a structured decision point that asks whether a change, access path, identity, or dependency creates unacceptable exposure, then records the reasoning behind acceptance, mitigation, or rejection.

For IAM and NHI governance, that means the review should be able to explain why access is justified, what secrets or privileges are involved, and what business function would be affected if the control failed. It is as much about documenting accountability as it is about identifying the risk itself.

Where Risk Review Fits in Security Governance

Risk reviews sit between design and deployment, or between continued use and renewal. They give organisations a way to pause when a system change, new integration, or elevated access path introduces something that standard controls do not fully cover.

In practice, the review becomes the bridge between technical facts and governance decisions: whether residual risk is tolerable, whether additional controls are needed, or whether the activity must not proceed. A good review is explicit about ownership, scope, and the condition that triggered it.

What Gets Examined During the Review

A meaningful review looks at the full exposure picture, not just the headline issue. That usually includes access scope, secret handling, privilege level, internet exposure, control gaps, logging coverage, recovery options, and the downstream impact if the change is abused or misconfigured.

It should also test whether the proposed state creates concentration risk, such as a single credential or integration becoming a critical dependency. Where the review involves machine or service access, the question is often whether the identity has more authority than the task actually requires.

How Risk Review Supports Decisions and Evidence

The value of a risk review is that it creates a defensible record. It shows who assessed the issue, what evidence was considered, what mitigation was required, and what residual risk was approved or rejected. That record matters when the same decision pattern is repeated later.

For teams building repeatable governance, a Risk Register Template is useful because it turns review outcomes into a consistent structure for risk statements, owners, treatment, residual risk, and review cadence.

Risk and Threat Considerations

Risk review failures usually show up as either over-approval or under-documentation. If the review is too shallow, excessive access, exposed secrets, or weak compensating controls can move a known issue into production with no clear acceptance trail.

Failure mechanism: The control fails when reviewers treat the process as a formality, ignore residual exposure, or approve a change without understanding the access path, trust boundary, or operational dependency being introduced.

Impact: That can lead to unauthorized access, compliance findings, untracked exceptions, delayed containment, or a control gap that persists until the next incident or audit.

Standards & Framework Alignment

This section maps relevant standards and security frameworks to the operational risks and controls described in this guidance.

NIST SP 800-53 Rev 5 and CIS Controls v8 set the technical controls, while ISO/IEC 27001:2022 defines the regulatory obligations.

FrameworkControl / ReferenceRelevance
NIST SP 800-53 Rev 5RA-3 — Risk AssessmentRisk review is the control activity this assessment requirement describes.
Recommendation — Perform structured risk assessments before approving changes or access paths.
ISO/IEC 27001:2022A.5.8 — Information security in project managementRisk review is embedded in change and project decisions before release.
Recommendation — Embed risk review checkpoints into project and change approval workflows.
CIS Controls v8CIS-5 — Account ManagementRisk review evaluates access scope, privilege and account exposure.
Recommendation — Review account and privilege decisions before granting or keeping access.

Practitioner Guidance

Governance implication: A risk review should have a clear owner, a defined trigger, and a consistent standard for what counts as acceptable residual risk. Without that, approval becomes subjective and exceptions multiply quietly.

What to watch for: Repeated approvals for the same class of issue usually mean the organisation is learning the symptom, not fixing the cause. If the same access, secret, or dependency keeps reappearing in reviews, the control design needs attention, not just another sign-off.
